Name Change

    1. If your current passport has been issued less than one year ago, complete, sign and submit the DS-5504 Online Application application online (Apply for a Passport Book). Print the DS-5504 form that is created for your at the end.
    2. If your current passport has been issued more than a year ago, please complete, sign and submit the DS-82 Online Application, include two passport style photos.
  2. Submit the original copy of the legal document that verifies your name change, such as a marriage certificate or a court order.

Expedited service takes approximately ten business days to complete. For a Rush, Will Call service, a flight itinerary or Business letter stating the urgency of this immediate international travel is required. This process generally takes about three days to complete.
