  The Visa Network Order Form is vital whenever you send us your Passport for visa processing. The information provided by the order form will help us determine how to serve your Visa and Passport needs to the fullest extent.
Intel employees are required to go to Intel's visa assessment tool to determine the appropriate visa. Visa is determined based on activities not length of stay. If a business visa is required, please return to this website to process your paperwork. Intel Corporation employees: Please provide us with credit card number and expiration date on the order form. You will receive a hard copy of the invoice for your expense report along with your travel documents once the process is complete. Please print and complete this form, and include it with your Passport and accompanying documents. We have made the Order Form available in different file formats below.
