• have not had a US passport before but are US born or are naturalized citizens
• were under 16 of age when passport was issued, even if it is for renewal
• passport expired more than 5 years ago
• passport that was lost or stolen, click here for additional requirements.

Checklist:

• Complete the DS-11 application online (Apply for a Passport Book), click "Create Form", download and print two copies, DO NOT SIGN IT
• Go to a Passport Acceptance Facility (County Court House or Post Office only)
  Take the following:
  1. 1 Passport Photos; taken in the past three months. (Passport pictures should not contain glasses! Passport pictures with glasses will not be accepted by the government.)
  2. Proof of US Citizenship: (a) certified birth certificate, issued by state or county health department or vital statistics bureau. Please note that hospital certificate is not acceptable. (b) Naturalization certificate (c) old expired U.S. passport.
  3. Photo ID, i.e. Driver's license
  4. Blank Checks: Payable to the "U.S. Department of State"; $170 and $25 service fee for adults and $140 and $25 service fee for minors. Do not write checks until instructed by the clerk.
  5. Take the Visa Network letter of authorization. each traveler must fill it out by hand. Indicate Visa Network as the courier company name.

     Please hand write Visa Network for "Courier Company Name" on this form.
     Please check the only the first two boxes! Do not check the third box, per the Passport agency.
     
     Letter of authorization for minor applicants.
     (This LOA is only for Minor applicants, both parents must sign the same Letter of Authorization. )(This letter is only for US Passport Process only, it is not intended for any visa application process.)
     Please hand write Visa Network for "Courier Company Name" on this form.
     
     
  6. Take the Permission to Process U.S. Passport Form with you so they will release the sealed envelope to you.
  7. If you require rush processing make sure to include a flight itinerary showing a departure date in the next two weeks.
     
     
  • Sign one set of the application in front of the Clerk.
  • Notify the clerk that you are using an expediting service (Visa Network) to "hand carry" your application. They will put all the above documents inside a sealed envelope. If the clerk does not feel at ease about releasing the sealed envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent's manual, "Hand-carrying of Executed Applications". (Do not open this sealed envelope or the application will be invalid)
• Attach the second set of DS-11 form to the outside of the sealed envelope.
• Send the sealed envelope, along with our Order Form to Visa Network.

Please make note of the following:

• All DS-11 passport applications submitted to the Passport Agency by Visa Network must be completed online, downloaded, and printed (one-sided). It must contain the 2-D barcode on the top left hand corner and on the bottom right hand corner.
  The 2-D barcode pages must be clear and sharp in black print on white paper. The paper must be 81/2 inches by 11 inches, with no perforations, one-sided. Applications printed on thermal paper, dye-sublimation paper, any glossy paper, or on a dot matrix printer will not be accepted.
  
  
• Non-rush (expedited) service takes approximately ten business days to complete.
  
  
• For rush service a flight itinerary showing a departure date within the next 5 days or a business letter stating the urgency of this immediate international travel is required. This process takes about three business days to complete.
  
  
• For emergency same day service, please call Visa Network (415)693-0330. Limited number of “same day” processing requests available, must call to secure a slot with Visa Network.
