U.S. Passports for Minors-Special Circumstances


For those who:
  • Wish to obtain a 1st time passport or passport renewal for Minors. All children regardless of age (including infants) must have their own passport. Minor Children must apply in person, with special requirements for children under age 16. Passports for children are issued for five years if applied for before age 16; this process must be repeated each time a new passport is needed.

Important Additional Information:

All minor applicants must apply as a 1st Time Passport. Make sure to take note of the information below, and then follow all 1st Time Passport instructions.
*Passport pictures should not contain glasses! Passport pictures with glasses will not be accepted by the government.

For All Minors under Age 16:

  • Each minor child shall appear in person.
  • All applications for children under 16 require either parents’ or legal guardian's consent.
  • For Minors under Age 16 you must establish proof of citizenship AND proof of relationship. Previous U.S. passports are not acceptable as proof of relationship to the applying parent(s)/guardian(s).

Present Parental Application Permission Documentation

  • Both parents must appear together and sign or
  • One parent appears, signs, and submits second parent's notarized statement of consent authorizing passport issuance for the child (a notarized Form DS-3053, Statement of Consent: Issuance of a Passport to a Minor Under Age 16, or a notarized written statement with the same information on a sheet of paper from the non-appearing parent) or
  • One parent appears, signs, and submits primary evidence of sole authority to apply.
  • Complete Visa Network letter of authorization.(This letter is only for US Passport Process only, it is not intended for any visa application process.)
    Please hand write Visa Network for "Courier Company Name" on this form.
    Please check the only the first two boxes! Do not check the third box, per the Passport agency.

For All Minors Ages 16 & 17:

  • Each minor child shall appear in person.
  • At least one parent (with Identification) should go with the minor. Minor should bring identification also if possible.
NOTE: Please do NOT sign Form DS-11 the Passport Acceptance Agent instructs you to do so. If your child is unable to sign his or her own name, a parent may sign for the child by writing the child's name on the signature line and signing the parent's name above the child's name. Example:
