Passport for Minors

The Passport for Minors service is created to walk anyone through the steps necessary to obtain a new passport for a child or minor. Everything needed is explained below. Please note: all applicant’s along with legal guardian(s) must first apply at an Acceptance Facility before sending documents to Visa Network.

INFO FOR MINORS UNDER AGE 16

  • Each minor shall appear in person at an Acceptance Facility
  • All applications for children 16 and under require either parents’ or legal guardian’s consent.
  • For Minors under Age 16, you must establish proof of citizenship and proof of relationship with parents. Previous U.S. passports are not acceptable as proof of relationship to the applying parent(s)/guardian(s).
  • Both parents must appear together and sign or:
    • One parent appears, signs, and submits second parent’s notarized statement of consent authorizing passport issuance for the child (a notarized Form DS-3053, Statement of Consent: Issuance of a Passport to a Minor Under Age 16, or a notarized written statement with the same information on a sheet of paper from the non-appearing parent)
    • One parent appears, signs, and submits primary evidence of sole authority to apply
  • Do not sign Form DS-11 till the Passport Acceptance Agent instructs you to do so

Notify the clerk you are using an expedited service (Visa Network). All contents will be placed into a sealed envelope. If the clerk does not feel at ease about releasing the sealed envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent’s manual, “Hand-carrying of Executed Applications”. Do not open this sealed envelope or the application will be invalid.

Include a copy of the DS-11 and Order Form outside of the sealed envelope for delivery.

INFO FOR MINORS AGES 16 & 17

  • Each minor child shall appear in person at an Acceptance Facility
  • At least one parent (with Identification) should go with the minor. Minor should bring identification also if possible.
  • Do not sign Form DS-11 till the Passport Acceptance Agent instructs you to do so. If your child is unable to sign his or her own name, a parent may sign for the child by writing the child’s name on the signature line and signing the parent’s name above the child’s name.

STEP 1

Gather the Checklist

 

  1.  LETTER OF AUTHORIZATION
    • Choose the appropriate LOA
       Letter of Authorization for ages 16 and 17
      Letter of Authorization for minors under 16
        Check mark the top 2 boxes and write “Visa Network” next to Courier Company Name. It should all be handwritten by the applicant (if possible). Parents must sign the LOA below the minors signature. Parent may sign for the child. 
  2.  DS-11 ONLINE APPLICATION FORM
    • Complete Online and create PDF
    • The PDF will be downloaded. Print two copies and don’t sign anything yet
    • You’ll be asked questions about processing and shipping methods. Any answer you choose is completely separate from our service and no payment will be asked. To finish the form- click on “Create Form”
  3.  PROOF OF US CITIZENSHIP
    • Certified Birth Certificate, issued by state, County Health Department, or Vital Statistics Bureau. Please note: a hospital certificate is not acceptable
    • Naturalization Certificate
    • Old, expired U.S. Passport + Birth Certificate Copy (If under 17)
  4.  TWO PASSPORT PHOTOS
    • 2×2″ in size, taken in the past three months, no glasses, with white white background
  5.  PHOTO ID (IF APPLICABLE)
    • Driver’s License or state I.D.
  6.  BLANK CHECK
    • Service fees for minors are 160 and 35 for clerk fee (varies). Some facilities do accept credit card.
  7.  FLIGHT ITINERARY
    • Required for expedite
    • Must show travel within 2 weeks when going to Acceptance Facility
  8. COPY OF DS-11
    • Agency requires a copy of the DS-11 to be attached outside of the sealed envelope. If the form was handwritten, please create a new form online.
  9. ORDER FORM
    • A form for Visa Network contact, shipping, and billing purposes. Include outside of the sealed envelope

STEP 3

Visit a Passport Acceptance Facility

 

Once you’ve gathered all your documents, visit a PASSPORT ACCEPTANCE FACILITY. Most require appointments, but some allow for walk-ins. Call (415) 693-0330 if you are having trouble locating one.

Just submit and email our Order Form to questions@visanetwork.com. Mention you’d like a pre-paid label on the email. It’s as simple as that. Shipping labels are non-refundable.

 

Notice

 

All children regardless of age (including infants) traveling overseas must have a passport. Minor Children need to apply in person, and there’s special requirements for children under age 16. Unlike adults, passports for minors are issued for five years if applied for before age 16. This process of going through a Passport Acceptance Facility first must be repeated each time a new passport is needed, and one more time as an adult of 18 years or older. Then, you may apply directly through Visa Network.

 

The Letter of Authorization must be handwritten by the applicant only (if possible) and both parents must sign the LOA unless only 1 parent is present. The pen used must be in black or blue ink and not interchanged. All fields must be completed including Courier Company Name- be sure to write “Visa Network.”

Passport Agency Amount

Paid directly at the Acceptance facility. The Agency amount will be enclosed inside the envelope. A clerk fee of $35 is paid to the Acceptance facility.
