Do It Yourself Passport

The Do It Yourself Passport Service is created to walk anyone through the steps necessary to obtain a new passport. Everything needed is explained below. Please note: All applicants under the age of 17 must appear at an Acceptance Facility along with their legal guardian(s). Included in the fees is a $60 expedite. Although it is not required, we highly recommend paying more for speed and security.

INFO FOR MINORS UNDER AGE 16

  • Each minor shall appear in person at an Acceptance Facility
  • All applications for children 16 and under require either parents’ or legal guardian’s consent.
  • For Minors under Age 16, you must establish proof of citizenship and proof of relationship with parents. Previous U.S. passports are not acceptable as proof of relationship to the applying parent(s)/guardian(s).
  • Both parents must appear together and sign or:
    • One parent appears, signs, and submits second parent’s notarized statement of consent authorizing passport issuance for the child (a notarized Form DS-3053, Statement of Consent: Issuance of a Passport to a Minor Under Age 16, or a notarized written statement with the same information on a sheet of paper from the non-appearing parent)
    • One parent appears, signs, and submits primary evidence of sole authority to apply
  • Do not sign Form DS-11 till the Passport Acceptance Agent instructs you to do so

 

INFO FOR MINORS AGES 16 & 17

  • Each minor child shall appear in person at an Acceptance Facility
  • At least one parent (with Identification) should go with the minor. Minor should bring identification also if possible.
  • Do not sign Form DS-11 till the Passport Acceptance Agent instructs you to do so. If your child is unable to sign his or her own name, a parent may sign for the child by writing the child’s name on the signature line and signing the parent’s name above the child’s name.

CHECKLIST (For Minors, First Time or Lost Passport)

 

  1.  DS-11 ONLINE APPLICATION FORM
    • Complete Online and create PDF
    • The PDF will be downloaded. Print two copies and don’t sign anything yet
    • You’ll be asked questions about processing and shipping methods. Any answer you choose is completely separate from our service and no payment will be asked. To finish the form- click on “Create Form”
  2.  PROOF OF US CITIZENSHIP
    • Certified birth certificate, issued by state, County Health Department, or Vital Statistics Bureau. Please note: a hospital certificate is not acceptable
    • Naturalization Certificate
    • Old, expired U.S. Passport
  3.  TWO PASSPORT PHOTOS
    • 2×2″ in size, taken in the past three months, no glasses, with white white background
  4.  PHOTO ID (IF APPLICABLE)
    • driver’s license or state I.D.
  5.  BLANK CHECK
    • Service fees for minors are 180 and 35 for clerk fee (varies). Some facilities do accept credit card.

 

Once you’ve gathered all your documents, visit a PASSPORT ACCEPTANCE FACILITY. Most require appointments, but some allow for walk-ins.

 

 

PASSPORT RENEWAL CHECKLIST

 

  1.  DS-82 ONLINE APPLICATION FORM
    • Complete Online and create PDF
    • The PDF will be downloaded. Print two copies and don’t sign anything yet

  2.  ORIGINAL PASSPORT
    • Not expired longer than 5 years
    • Not damaged
  3.  TWO PASSPORT PHOTOS
    • 2×2″ in size, taken in the past three months, no glasses, with white white background
  4.  PHOTO ID (IF APPLICABLE)
    • Driver’s license or state I.D.
  5.  BLANK CHECK
    • Service fees for minors are 190 and 35 for clerk fee (varies). Some facilities do accept credit card.

Price does not include 3.5% bank fee or extra handling charges that might incur during the process. We don’t have hidden fees and we won’t charge extra without letting you know. Please include a credit card on the Visa Network Order Form. Alternatively, you may pay with cash, check or money order.
